Basically fine.  I just feel we need them to sign a statement to say that 
the details they have included are correct to cover ourselves should there 
be any "exaggerations" of qualifications, experience etc.

The categories I think are fine, and if you want another example for 
courses, how about Computer Programming?  It's a nice general description 
like Graphic Design.
